File a written fraud report with each of the three major credit bureaus. You can

Contact businesses in writing of fraudulent account(s) to close accounts and reopen
with new security measures, such as a password.

|
|
|
|
File a police report and record the precint number, officer name, date filed, and
case number. Follow up and obtain a copy of the police report.

|
|
|
Contact the Federal Trade Commission and complete the I.D. Theft Affidavit. You
